In network with most major plans

We bill most major commercial plans and state and federal programs. If we are out of network with yours, our published cash-pay rates apply — no surprise balance later. Call (541) 555-0142 and we will check your benefits before your first visit.

What we treat

Six programs, built around how Central Oregon actually moves.

Every plan of care starts with a full evaluation and a written timeline. You will know what we are treating, what the next four weeks look like, and what has to be true before you go back to sport.

Orthopedic physical therapy

The everyday stuff that stops you: low back and neck pain, tendinopathy, shoulder impingement, hip and knee pain, ankle sprains, and the flare-ups that keep coming back.

Typically 6–10 visits 45-min sessions

Post-surgical rehabilitation

ACL reconstruction, rotator cuff repair, hip and knee replacement, meniscus and labral repairs. We follow your surgeon's protocol and coordinate directly with their office at each milestone.

3–9 months by procedure Protocol-guided

Running gait analysis

Slow-motion video from four angles on our treadmill, plus cadence, contact time and load testing. You leave with two or three cues that matter — not a list of twenty.

75-min single session Video report emailed

Climbing & ski injury rehab

Finger pulleys, elbow and shoulder overuse from Smith Rock seasons, plus knee injuries off Bachelor. Loading progressions built for hangboards, skin tracks and moguls.

Typically 5–12 visits Sport-specific loading

Dry needling & manual therapy

Trigger point dry needling, joint mobilization, soft tissue work and cupping — used to open a window for movement, then paired with loading so the change sticks.

Add-on or standalone Certified providers

Return-to-sport testing

Objective hop, strength and single-leg control testing scored against your uninvolved side. We send the report to you and your surgeon, and we tell you plainly if you are not ready yet.

90-min test session Written clearance report
Return-to-sport program

Four stages, and you always know which one you are in.

Most patients move through all four. Timelines shift with the injury and the surgery, but the criteria to advance never move — you earn the next stage with measurable numbers, not with the calendar.

Start with an evaluation
01
Weeks 0–2

Assess

A full 60-minute evaluation: history, movement screen, strength and range measurements, and a clear working diagnosis.

  • Baseline strength and range numbers
  • Written plan and visit estimate
02
Weeks 2–6

Restore

Calm the irritated tissue, get range and basic strength back, and rebuild the movement pattern before we add real load.

  • Manual therapy and dry needling as needed
  • Home program of 3–4 exercises, not 15
03
Weeks 6–14

Load

Progressive strength in the gym, then plyometrics and change of direction. This is the stage most people skip on their own — and the one that prevents the repeat injury.

  • Strength retested every 3 weeks
  • Impact and cutting added by criteria
04
Week 14+

Return

Formal return-to-sport testing, a phased return schedule for your first weeks back, and a maintenance plan so the work holds through the season.

  • Hop and strength symmetry scored
  • Report sent to you and your surgeon
Our team

Four clinicians. You see the same one every visit.

No aides running your session and no double-booking. Whoever evaluates you is the person who treats you through discharge.

Dr. Maren Ostlund

PT, DPT, OCS
Orthopedics & post-surgical

Founder and clinic director. Board-certified orthopedic specialist who opened Cascade in 2016 after nine years in hospital-based rehab.

Dr. Theo Vance

Dr. Priya Raghunathan

Manual therapy & dry needling

Fellowship-trained in manual therapy and our most requested clinician for stubborn neck, jaw and mid-back pain.

Dr. Case Whitmore

PT, DPT, CSCS
Climbing, ski & shoulder

Your first visit

Sixty minutes, one clinician, and a plan before you leave.

Booking to first appointment is usually two to three business days. Here is exactly what happens, so nothing about it is a surprise.

STEP 01

Book & check benefits

Book online or call. We verify your coverage and text you your expected cost before you arrive.

  • Same-week openings most weeks
  • No referral required in Oregon
STEP 02

What to bring

Paperwork goes out by email the day you book, so most people arrive with it already done.

  • Photo ID and your insurance card
  • Clothes you can move in
  • Any imaging or op notes you have
STEP 03

The evaluation

A full hour in a private room. Fifteen minutes of history, then hands-on testing and measurement, then we start treatment the same day.

  • Range, strength and load testing
  • Treatment begins on day one
STEP 04

What you leave with

A working diagnosis in plain language, a visit estimate, and a home program in an app with short video for each exercise.

  • 3–4 exercises, not a packet of 15
  • Your next two visits already scheduled

Follow-up visits run 45 minutes and are one-on-one with your clinician. Most patients come twice a week for the first two to three weeks, then taper as strength and confidence come back.

If we bill your plan, your cost is whatever your plan says — copay, coinsurance or deductible. If you would rather skip insurance entirely, these are the rates, and they do not change.

Paid at time of service. We provide an itemized superbill you can submit to your plan for out-of-network reimbursement.

Patient stories

What people say after discharge.

Every patient's course is different, and we will always tell you honestly what we expect yours to look like.

"I had been managing the same hip pain for two summers. The gait session found it in twenty minutes, and the fix was two cues and a strength block. I ran the Horse Butte loop pain-free in week seven."
Danielle R.Trail runner, Bend
"Rotator cuff repair in April. They coordinated with my surgeon's office at every milestone and never let me rush a stage. I was back on patrol for opening weekend with a shoulder that felt like mine again."
Marcus O.Ski patroller, Redmond
"The return-to-sport testing was the part nobody else offered. Seeing my numbers side by side made it obvious I wasn't ready in month six — and obvious that I was in month eight."
Jenna P.Club soccer, ACL rehab
"A pulley strain three weeks before a Smith trip. They gave me a loading plan instead of telling me to stop climbing, and I taped and sent at a grade below all week without making it worse."
Ray T.Climber, Terrebonne
Questions

The things people ask before booking.

No. Oregon has direct access, which means you can be evaluated and treated by a physical therapist without a physician referral. A few insurance plans still require one for billing purposes, so we check that for you when we verify your benefits — before you spend a dollar. If your plan needs a referral, we will tell you exactly what to ask your doctor's office for.

Your first visit is 60 minutes and follow-ups are 45. Running gait analysis takes 75 minutes and return-to-sport testing takes 90. Every minute is one-on-one with your clinician in a private treatment room — we do not hand you off to an aide halfway through.

We are in network with most major commercial plans as well as state and federal programs, and we accept auto and workers' compensation claims. Rather than guess from a list, send us your plan details and we will verify your benefits and text you your expected per-visit cost, usually the same business day. If we are out of network, our published cash-pay rates apply and we provide a superbill for reimbursement.

Whatever you would wear to the gym. For a lower body problem, bring shorts so we can see the knee and hip move. For a shoulder or neck problem, a tank top or a shirt that comes off easily helps. If you are coming for gait analysis, bring the shoes you actually run in — worn ones tell us more than new ones. We have a changing room and lockers if you are coming from work.

Our average across all patients is a little over eight visits. Straightforward orthopedic problems often resolve in six to ten; post-surgical rehab runs months, not weeks, and follows your surgeon's protocol. You will get a visit estimate in writing at the end of your evaluation, and we revisit it out loud every few weeks. Individual results vary, and we would rather discharge you early than keep you on the schedule.

Yes — three of our four clinicians are certified in trigger point dry needling. Most people describe a deep ache or a quick twitch rather than sharp pain, and some muscle soreness for a day afterward is normal. It is always optional, we explain it fully before the first treatment, and we pair it with loading work so the change lasts instead of wearing off by the weekend.
